Evaluation request letter

Ask the school, in writing, to evaluate your child for special education. Fill in a few boxes and print, email or download the letter.

How it works

Private by design: what you type is saved in this browser on this device only. It is never sent to SPED Atlas or anyone else. On a shared or library computer, press Clear this tool when you finish.

Asking in writing gives you a dated record, and the timeline for the evaluation starts once you give consent. The school must either ask for your consent to evaluate or send you prior written notice saying why not. Hand it in and ask for a dated copy, or email it and keep the sent email.

The law: 34 CFR 300.301; 300.304(c)(4); 300.503; 300.504(a)

Your state may set a different timeline. See your state.
